The Release of Liability must be signed by anyone participating in equine activities at Patriot Farm at the Grand View facilities.  It must also be signed in order to be at Patriot Farm at the Grand View un escorted by an employee.

The Barn Rules must be signed in order to be at Patriot Farm at the Grand View un escorted by an employee.

Board contract must be signed prior to boarding your horse at Patriot Farm at the Grand View.  All forms are linked for your convenience.
